IMPORTANT MEMBERSHIP TERMS
A 12-month commitment is required. All treatments must be completed within 12 months from the purchase date. Membership dues will automatically be drawn every month. Membership cannot be canceled/terminated early. Memberships are non-transferable and can only be used by the named member. Treatments cannot be modified or substituted. Members will be notified in advance of any changes to the membership terms. Membership fees are non-refundable, even in the case of early termination.
