What is pellet bio-identical hormone replacement therapy?

What are pellets?

Each pellet implant is made up of a bio-identical hormone, either testosterone or estradiol, which is the exact chemical formula and molecular structure of what your body produces.  They are derived from natural plants that are pressed into small solid cylinders, larger than a grain of rice and smaller than a Tic-Tac. In the United States, most pellets are made by compounding pharmacists since big pharmaceutical companies can’t profit from the production of unpatentable, natural substances. At Colonial Aesthetics & Wellness, we did our research and selected a compounding pharmacy that produces the best pellets in the country.

Why choose pellets?

Pellets deliver consistent, healthy levels of hormones for approximately four months in women and five months in men.  The pellets dissolve completely on their own during this time.  They avoid the fluctuations of hormone levels seen with every other method of delivery.

Hormone balance is important for optimal health and prevention of disease and chronic illnesses like Alzheimer’s and Parkinson’s. Pellets do not increase the risk of blood clots or other complications that which conventional/synthetic hormone replacement therapy has been associated.

In studies comparing pellets to conventional hormone replacement therapy, pellets are superior for the relief of menopausal symptoms, maintenance of bone density, restoration of sleep patterns, and improvement in sex drive, libido, sexual response, and performance.

Testosterone delivered by a pellet implant has been used to treat migraine and menstrual headaches and help with vaginal dryness, incontinence, urinary urgency, and frequency. In both men and women, testosterone has been shown to increase energy, relieve depression, increase sense of well-being, relieve anxiety, and improve memory and concentration. Testosterone delivered by pellet implant increases muscle strength and bone density and decreases fat mass.

Patients who have not had success with other types of hormone therapy have a high success rate with pellet therapy. There is no other method of delivery that is as convenient, effective, and safe as pellets, which have been used in both men and women since the late 1930s.

What are the advantages of pellet therapy?

Advantages include:

  • relieving menopause symptoms

  • improving low sex drive

  • improving energy and mood

  • reducing body fat

  • protecting against health conditions, such as heart disease and osteoporosis

  • direct absorption of hormones into the bloodstream, avoiding the liver and gastrointestinal system

  • control over the release of hormones so the body can absorb more or less as and when it needs

  • continuous availability of hormones

  • no need to apply topical creams/gels each day or use patches

How are pellets inserted and how long until a patient feels better?

The provider will numb the skin and make a small incision, usually into the hip area. She will then insert one or more hormone pellets underneath the skin and close the incision with steri-strips. The steri-strips can be removed after 5-7 days. The hormones are gradually delivered into the bloodstream. Some patients begin to feel better within a couple of days, while others may take a few weeks to notice a difference.

How long do the pellets last?

Pellets usually last between 3-5 months in women and 4-6 months in men.  The pellets do not need to be removed; they completely dissolve on their own.

How are the hormone doses determined?

Hormone levels will be drawn and reviewed before each therapy session. Individualized dosing is calculated from your lab values, height, and weight.  This type of hormone dosing guarantees that you will receive the exact amount that your body needs.

Do female patients need progesterone when they use the pellets?

Women who are treated with testosterone pellets alone (no estradiol) do not require progesterone therapy. However, if estradiol is prescribed to a woman who still has her uterus, progesterone is also needed. This is to prevent the proliferation (stimulation) of the uterine lining caused by estrogen.

Is there a role for testosterone pellets in women?

Testosterone pellets may be used in premenopausal women. Testosterone has been shown to relieve migraine and menstrual headaches, help with symptoms of PMS (premenstrual syndrome), relieve anxiety and depression, increase energy, help with sleep, and improve sex drive and libido. If a premenopausal woman has a testosterone pellet inserted, birth control is recommended, as there is a theoretical risk of masculinizing a female fetus.
